Abstract

Patent ductus arteriosus (PDA) is a rare congenital heart disease in adults. Situs ambiguous with polysplenia (SAP) is also a rare disease entity, characterized by complex anomalies involving not only intraabdominal organs, including multiple spleens, but also cardiovascular system. We report a rare case of 30-year-old female with incidental multiple spleens, double sided inferior vena cavas with azygous continuation at renal vein level, semi-annular pancreas, centralized liver, and right-sided stomach, which is consistent with SAP. She also had a PDA, an unusual combination with SAP. Situs ambiguous with polysplenia syndrome has been occasionally reported. However, among the accompanied cardiovascular anomalies, PDA is extremely rare. Unlike other structural anomalies of situs ambiguous, PDA may alter the hemodynamics of the patient, therefore treating PDA is recommended.
